So, you want to be a part of a camp leadership team? In addition to completing the 'normal' camp form, there are some additional forms that must be completed:

• Team Member Application Form: This form is to be completed by you and returned with the 'normal' camp form.
  
  
• Referee Questionnaire: You must ask two responsible people to complete a referee questionnaire and return it directly to The Camping Ministry Coordinator. The two people must know you well but not be family members.
  
  
• You must also complete either an 18+ Medical Form (if you are aged 18 or over) or an U18 Medical Form (if you are under 18).
